Timothy Joseph Polak, Jr., 22 years of age, passed away Thursday January 7, 2010 at Robinson Memorial Hospital in Ravenna, Ohio.  Born on July 3, 1987 in Cleveland, Ohio, Tim was the son of Timothy J. and Almarine (Seese) Polak, Sr.  A resident of West Salem the past 15 years, he had been employed as a roofer at Twin Peaks Roofing in West Salem.  Tim loved spending time with his children and friends, and was always there for anyone who asked for his help at anytime.  Survivors include his children Chase and Rylee both of Wooster and Carson of Wadsworth, his mother Almarine (Dailey Davis) Polak of Streetsboro, his father Timothy J. Polak, Sr. of West Salem, his sister Tiffany Polak of Ashland, grandparents John R. and Dorothy E. Seese of Orwell, Ohio and many loving aunts, uncles and close family and friends.  Calling hours will be Monday January 11, 2010 from 5:00 P.M. - 7:00 P.M. at the Green Family Funeral Home & Crematory Service in Mantua, where funeral services will begin at 7:00 P.M.  Tim will be laid to rest at Congress Cemetery in Congress Village, Ohio.
